okayy.
hmmm heres the new tepid sponging notes that aic typed out:

TEPID SPONGING
9 flannels
2 wet cloths (for cleaning)
A basin of water
1 dry cloth (dap dry)
1 big bath towel
Incontinent sheet (amnt depends on the size pf patient)
A new set of pyjamas
A basin to collect the flannels


NO.4:

Judge, we are now going to proceed with tepid sponging. This is to reduce the elevated body temp of the patient and only done when the temp is 38.5 degrees or above as the normal temp is 36 to 37.4 degrees. We have to do tepid sponging bcos the patient is immobile/ unconscious.
Before we continue, I’ll switch off the fans so that the patient will not catch a cold as we will undress her later.
Meanwhile, my partner will place incontinent sheet on the bed as the bed shet is to thin to absorb all the water. Next, she will pull the screens, allowing the patient to undress herself while she waits outside.
Next, my partner will place cold compresses. 1 on the neck, 2 on each side of the neck, another 2 on the axillas and another 2 on the groin. These are the places where the major blood vessels lie.
We’ll ensure that the cold compresses are changed at frequent intervals to retain the coldness and also check if the water turns warm. If it does, we’ll change it. At the same time, we’ll also ensure the process does not exceed 25 min.


We’ll look out for any shivering and will stop immediately if there is any because shivering produces heat and will elevate the body temp instead.
Next, we will use long sweeping downward strokes to sponge the arms, front of the body and legs. My partner will pass me the flannels as we go along. We’ll wash the wet towels in the basin after cleaning each part.
We will ensure not to rub agst the skin as friction will elevate the body temp and cause pressure sores to form. We will expose the patient as little as possible to protect her modesty.
We will also leave a film of water behind for evaporation.
We’ll turn the patient to my side and
clean the patient’s back.
Lastly, we will dap the patient dry and dress her. Meanwhile, my partner will take out the incontinent sheet.
Then, we’ll switch on the fans. We’ll come back half an hour later to monitor the patient’s temp to check for the effectiveness of tepid sponging.

, we’re going to put cold compresses on you now so as to reduce your body temp.
(pls do not be alarmed as now I’m going to lower down the upper part of the bed, at the same time elevating the height of the bed)
, I’m going to place incontinent sheet beneath you so as to absorb the excess water.
pls get undressed. I’ll leave the bath towel here and I’ll wait for you outside ok ? Call me if you need help and when you’re done.
Now, my partner and I are going to place cold compresses at your forehead, neck, armpits and thigh area to reduce your body temp.
Don’t worry, we’ll only do this for less than 25 mins to prevent chill.


If you feel too cold, pls inform us ok? Because if you shiver, heat will be produced and increase your body temp.
Okay, we will remove your cold compresses and sponge your body now.
, pls do not be alarmed to feel a film of water after we finish sponging as this is required for evaporation.
Now, we’ll turn you to my partner’s side to clean your back too.
I’ll dap your body dry and dress you.
*if patient is willing to do it herself, let her do it.
Okay we are done with tepid sponging and will come back half an hour later to check your body temp again.


* if bed making has to be done after tepid, stick to the old notes.
